Time Is Brain: Why Aren’t Mobile Stroke Units Everywhere?

Dr. May Nour’s team has spent nine years bringing stroke care to patients before they reach the hospital. Every minute treatment is delayed can affect whether someone recovers or lives with permanent disability. A mobile stroke unit brings a medical team, CT scanner, and treatment directly to the patient rather than waiting for the patient to reach the hospital — but someone still had to assemble the medical team, find the right equipment, work across fire departments and county agencies, and determine how a hospital could deliver stroke care through the 911 system. Dr. Nour took that on. Nine years later, the evidence is clear: treating stroke patients earlier can significantly reduce disability. The harder question is why our healthcare system still has no reliable way to support that care before a patient reaches the hospital.

A Wildfire, Cancer, and AI Sent Him Into Founder Mode.

When the Palisades wildfire destroyed Steve Brown’s home, the displacement forced him into a new health system, where doctors found an aggressive blood cancer that had been missed for months. Brown is a Stanford-trained physicist and computer scientist who spent decades building technology in Silicon Valley and helped pioneer remote patient monitoring before it became standard care. He approached his diagnosis like a founder, running hundreds of medical documents through a swarm of AI agents examining his case from different clinical perspectives. What they surfaced wasn’t a new test result. It was a plasma cell problem already sitting in his lab notes. That discovery led Brown to build CureWise and confront a larger question: What could AI find in our medical records that the healthcare system has overlooked?

Betting on clinical trials is here. He’s trying to stop it.

We weren’t planning this episode. But when our own co-host became part of a national debate over betting on clinical trials, we decided the conversation couldn’t wait. In this surprise special release between our regular episodes, Timothy Wheeler interviews co-host David Tsai about the stand that brought The New York Times and NPR to his door. When Kalshi opened prediction markets on clinical trials and drug approvals, Tsai started a petition to stop the betting. Colleagues warned him about taking on a powerful company. He went ahead anyway. Could these markets make drug development more transparent, as their supporters argue, or undermine the trust that makes clinical research possible? And what happens when someone decides that staying quiet is no longer an option?

Her work grew from spending the first nineteen years of her life being told her symptoms weren’t real. In this conversation, she explains why medical gaslighting happens, the cost of not being believed, and why patients have more power to change the system than ever before.

Why did a doctor buy a 600,000-square-foot hospital for $1?

Days before this conversation was recorded, Dr. Tim Nelson acquired a 600,000-square-foot hospital — for one dollar. He left one of the country’s leading medical institutions after spending 20 years learning how to rebuild hearts that surgeons could only repair. This is a conversation about what he was willing to risk, why promising science moves so slowly, and whether a new model can bring potential cures from the laboratory to patients faster.
